Part Number:PGA280

While evaluating the PGA280 as the front end of an ADS1263, I am currently getting consistent errors marked by register4: 0x49. This shows me there is an issue in the input amplifier.

Unfortunately the pga280 datasheet names the error bits but not the exact conditions (figure 49 helps but is not very precise)...
Is it possible to get further info on the thresholds or exact conditions for triggering each error bit of register 4?

For instance, I am currently getting IAR and IOV errors. What is the threshold for IAR, is it (vin < VSON+2.5) and (vin > VSOP-2.5)? Is it the same threshold for IOV?

Note1: The input to the pga280 is a constant DC voltage. As you can see on reg11 below I am using the longest error flag suppression time, so this is not an spurious condition.
Note2: after reading the error bits I write 0xff to all of them in order to clear. I can see that the clearing takes effect as the device boots with 0x4D at times and changes to 0x49 after the clearing.

Pin voltages:

4: VSOP=3V3
5: VSON=0 (have also tried with -2.6V)
6: VSP=11V
11: VSN=0V (have also tried with -2.6V)
12: DGND = 0V
13: DVDD = 3V3

3: 0V6 to 1V3 (comes from ADS1263)

7,10: INP1=INN2 : 1.27V (tried range of 0.2 to 3.0 volts, producing same error)
8,9: INP2=INN1: 0.32V (should not matter as b1 and a2 are open and c2 is closed, see reg6 above)
