cc2510 PCB antenna

Hi, questions regarding the cc2510 PCB antenna, as shown on the picture above taken from the TI design note, the two ends seem to be in the "ground layer 2", but then there is "via to ground" on one end, that confused me.  Are those two ends in the ground layer or the top layer?

I am using a 2-layer instead of the 4-layer board, with the bottom layer being the ground layer.  Can I simply put the antenna on the top layer, and connect one end through via to the ground layer, and the other end (feeding point) to the resistor on the top layer?  I simply don't understand why there is a "ground layer 2" at the bottom half of the picture shown above, does it mean the ground pour?

Also, what do "D1", "D2", and "D3" refer to?  Do they mean the distance from the antenna to the edge of the PCB board?  If that's the case, then the board size will be defined by the antenna size, doesn't really make whole lots of sense.
