RTOS/CC2640: Run software without JTAG cable attached and reset when power provided?

Hello everyone,

I'm working a custom board that has CC2640F128 as my previous post here. However, there is a strange issue I'm having, the software runs when I start the program with the JTAG cable attached. When I remove the JTAG cable, it still runs (because there are blinking LEDs in my software). But, when I remove the 3V battery power and reinsert it, the LEDs stop blinking so I assume the software crashed or not run anymore.

I don't know if it's relevant, but when I tried to hit the reset button on the board with the JTAG cable attached, I got the following error,

Cortex_M3_0: Can't Run Target CPU: (Error -2134 @ 0x0) Unable to control device execution state. Reset the device, and retry the operation. If error persists, confirm configuration, power-cycle the board, and/or try more reliable JTAG settings (e.g. lower TCLK). (Emulation package 6.0.628.1) 
Cortex_M3_0: JTAG Communication Error: (Error -1170 @ 0x0) Unable to access the DAP. Reset the device, and retry the operation. If error persists, confirm configuration, power-cycle the board, and/or try more reliable JTAG settings (e.g. lower TCLK). (Emulation package 6.0.628.1) 

I tried to run the program in both debug and release mode. Some posts on google suggested this issue might be due to not flashing the code to the internal memory? Is that true, and if so, how do I do that? I tried the flash programmer 2 and flash the software .o file to the board and the issue still remains.
