TPS62150 - Does not supply the rated 1Amp of current

I tried everything so far, but this power supply is setup for 5.08V but it outputs the following:

With 20 ohm load supplies 30mA @2.1 Volts but it should supply 250mA @5.08 Volts

with 10 ohm load supplies 54mA @1.9 volts but should supply 500mA @5.08 Volts

Schematic is below:

I am supplying max of 4 Amps @7.2Volts to PVIN.  I am using Recommended inductor in page 18 of specification data sheet, the XFL3012-332MEC.  Am I doing something Wrong?
