ADS1220: ADC is damaging

I  am using ads1220 with 5v supply for both analog and digital part. The reference is 4.096V. My inputs are damaging time to time. The input voltage is maximum 4.5V. How to protect the adc from damaging. I have used around 50 nos out of that 5 got damaged.
